HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Commitment

Flow Wellness is committed to protecting the privacy of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable state law.

How We May Use and Disclose Your PHI

Treatment. We use and disclose your PHI to provide, coordinate, or manage your healthcare and any related services. This includes consultation, treatment, and follow-up.

Payment. We may use and disclose PHI to obtain payment for services. Flow Wellness operates on a self-pay model and does not bill insurance, but PHI may be shared with payment processors as needed.

Healthcare Operations. We may use and disclose PHI for operations such as quality assessment, training, and clinical reviews.

Required by Law. We will disclose your PHI when required to do so by federal, state, or local law.

Business Associates. We use third parties (Jane App, our compounding pharmacy partners) under signed Business Associate Agreements (BAAs) to support our services. These partners are contractually required to protect your PHI to HIPAA standards.

Authorization Required

Other than the uses described above, we will obtain your written authorization before using or disclosing your PHI. You may revoke an authorization in writing at any time.

Your Rights

  • Right to access and receive a copy of your medical record
  • Right to request amendment of your medical record
  • Right to request restrictions on certain uses and disclosures
  • Right to request confidential communications
  • Right to receive an accounting of disclosures
  • Right to receive a paper copy of this notice
  • Right to file a complaint with the U.S. Department of Health and Human Services (Office for Civil Rights) without retaliation

Where Your Records Live

Your medical record is stored exclusively in Jane App, a HIPAA-compliant electronic health record platform. Flow Wellness does not store PHI on this website, in any marketing system, or on any third-party platform that is not BAA-covered.

Mobile App

The Flow Wellness mobile app does not collect, store, or transmit PHI. Protocol scaffolding (start date, dose log) lives only on your device. All clinical messaging happens inside the Jane patient portal, deep-linked from the app.

Changes to This Notice

We reserve the right to revise this notice. Revised notices will be posted on this page and made available on request.
